Epic's mission is to lead people into a life-changing relationship with Jesus Christ and Relief Teams help do that by providing short-term relief in care and emergency situations. Relief Teams offer the unique opportunity to stretch out a helping hand in a critical care moment and begin to build a bridge to lead someone to Jesus through serving. RELEASE AND WAIVER OF LIABILITY FOR VOLUNTEERS

This Release and Waiver of Liability (the “Release”) in favor of Epic Church, a Florida not-for-profit
corporation, its directors, officers, employees, and agents as well as fellow volunteer personnel or
primary beneficiaries of Epic Church (collectively, “Epic Ministries, Inc.”).
I/we desire that the Volunteer work as a volunteer for Epic Church and engage in the activities
related to being a volunteer (the “Activities”). I/we understand that the Activities may include
constructing and rehabilitating homes, coming into contact with hazardous materials, preparing
food, cleaning, and interacting with the public.
I/we do hereby freely, voluntarily, and without duress execute this Release under the following
terms:
1. Release and Waiver. I/we do hereby release and forever discharge and hold harmless Epic
Church and its successors and assigns from any and all liability, claims, and demands of whatever
kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer’s
Activities with Epic Church. I/we intend for this release to include fellow Volunteers and any
beneficiary of the Epic Church Activities. I/we understand that this Release discharges Epic
Church from any liability or claim that the I/we may have against Epic Church with respect to any
bodily injury, personal injury, illness, death, or property damage that may result from Volunteer’s
Activities with Epic Church, whether caused by the negligence of Epic Church or its officers,
directors, employees, or agents or otherwise. I/we also understand that Epic Church does not
assume any responsibility or obligation to provide financial assistance or other assistance,
including but not limited to medical, health, or disability insurance in the event of injury or illness.
2. Medical Treatment. I/we do hereby release and forever discharge Epic Church from any claim
whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service
rendered in connection with the Volunteer’s Activities with Epic Church.
3. Assumption of the Risk. I/we understand that the Activities may include work that may be
hazardous to the Volunteer, including, but not limited to, construction and rehabilitation, loading
and unloading, coming into contact with hazardous materials, food preparation, cleaning, and
transportation to and from the worksites. I/we hereby expressly and specifically assume the risk
of injury or harm in the Activities and release Epic Church from all liability for injury, illness, death,
or property damage resulting from the Activities.
4. Insurance. I/we understand that, except as otherwise agreed to by Epic Church, in writing, Epic
Church does not carry or maintain health, medical, or disability insurance coverage for any
Volunteer. Each Volunteer is expected and encouraged to obtain his or her own medical or health
insurance coverage.
5. Photographic Release. I/we do hereby grant and convey unto Epic Church all rights, title, and
interest in any and all photographic images and video or audio recordings made by Epic Church
during the Volunteer’s Activities with Epic Church, including, but not limited to, any royalties,
proceeds, or other benefits derived from such photographs or recordings.
6. Other. I/we expressly agree that this Release is intended to be as broad and inclusive as
permitted by the laws of the State of Florida, and that this Release shall be governed by and
interpreted in accordance with the laws of the State of Florida. I/we agree that in the event that
any clause or provision of this Release shall be held to be invalid by any court of competent
jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining
provisions of this Release which shall continue to be enforceable.
IN WITNESS WHEREOF, I/we execute this Release as of the day and year written below.

NATIONAL RELIEF TEAM MEMBER EXPECTATIONS

National Relief Team participation may require travel and overnight stays. By agreeing to the following, I affirm my commitment to uphold these expectations as a member of the team:

  1. Trip Purpose – I understand that I am participating in a National Relief Team trip to provide short-term relief with the purpose of building a bridge to lead people to Jesus. I agree to support this purpose through my attitudes and actions, and to conduct myself in a manner that represents Jesus well.

  2. Belief Agreement – I agree to support the mission, values, and beliefs of Epic Church.

  3. Team Player – I agree to be a team player by interacting with others in supportive and encouraging ways, maintaining a servant’s attitude, and remaining flexible as circumstances require.

  4. Respect – I agree to respect and follow the directions of the Relief Team Leadership Team and any partner organizations.

  5. Relationships – I agree not to engage in exclusive relationships during the trip. If my spouse or significant other is on the team, I will intentionally interact with all members of the team, not only with one another. If I am attracted to a teammate, I agree not to pursue a romantic relationship during the trip. I further agree to refrain from any activity that could be construed as romantic interest toward those we are serving. I agree not to be alone with anyone of the opposite gender (with the exception of my spouse).

  6. Safety – I agree to create and maintain a safe environment on the trip by upholding Epic Church’s standards and policies, including those regarding sexual misconduct.

  7. Substances – I agree to abstain from the consumption of alcoholic beverages, tobacco products, and illegal drugs during the trip, including travel to and from the destination. This includes marijuana/THC in any form, even if obtained by prescription or permitted under state law.

  8. Technology – I understand that this trip is an opportunity to serve and to build relationships with my team and our partners. I agree to limit my use of technology and remain present in each moment.

  9. Dress – I agree to dress modestly and follow the partner organization’s dress code policy.
